Benefits Counselor – Queens
The Benefits Counselor will assist consumers with a comprehensive intake, assessment of needs, entitlement benefits screening, information, and referrals, and ensure appropriate follow-up of services. In addition, consumers will have the opportunity to develop an independent living plan that documents their goals, steps to achieve them, services that will support them in seeking their goals, and outcomes. The benefits counselor will assist individuals in knowing their rights and understanding how to self-advocate. The counselor will engage in advocacy together with and on behalf of consumers, including representation in hearings and appeals. This position is currently hybrid but will be based at our Queens office.

HBCI Crisis Interventionist
CINDY is seeking compassionate and skilled mental health clinicians to join CIDNY’s Home Based Crisis Intervention (HBCI) team as Crisis Interventionists. This role provides intensive, short-term, home- and community-based crisis intervention to youth ages 5-20 with intellectual and/or developmental disabilities (I/DD) and co-occurring mental health needs who are at imminent risk of psychiatric hospitalization.

CIDNY is seeking a dedicated and experienced Licensed Clinical Social Worker (LCSW) or Licensed Mental Health Counselor (LMHC) to serve as the Clinical Supervisor for CIDNY’s Home Based Crisis Intervention (HBCI) program. The ideal candidate is a strong clinical leader with expertise in trauma-informed care, intellectual and/or developmental disabilities (I/DD), and child and family mental health, and is passionate about supporting youth and families in crisis. Healthcare Advocate / Attorney
The Health Care Advocate / Attorney will assist seniors and people with disabilities as they transition into and navigate Medicaid Managed Long-Term Care (MLTC). Attorney will be part of the Independent Consumer Advocacy Network (ICAN), the New York State Ombuds program for Managed Long-Term Care. The attorney will work with consumers to resolve issues with their MLTC plans.

NY Connects Benefits Counselor - Bronx
The NY Connects Program provides Information and Assistance and Person-Centered Options Counseling for individuals seeking access to Long-Term Services and Supports (LTSS). The Benefits Counselor assists consumers through information and referral services, intake and assessment, benefits eligibility screening, application assistance, and ongoing follow-up. Consumers may also develop person-centered independent living plans that identify goals, action steps, supports, and desired outcomes.

Peer Advocate
CIDNY is seeking a full time Peer Advocate for Open Doors Transition Center program that assist individuals wishing to transition from institutional settings to the community. The Peer Advocate will provide one on one peer support to individuals and families in skilled nursing facilities and developmental centers to share their independent living experiences to support community living option. Peer Advocates are people who receive(d) home and community-based services to help them live in the community. Peer Advocates with Developmental Disabilities are people who receive(d) services from the Office for People with Developmental Disabilities. Veteran Peer Advocates are veterans and/or military connected individuals with disabilities. Family Peer Advocates are family members or loved ones of people who live(d) in an institutional setting.

Transition Specialist
CIDNY is seeking a Transition Specialist for a new statewide initiative to assist individuals transitioning from institutional settings to the community. The Transition Specialist will facilitate and coordinate for individuals residing in institutions with a comprehensive plan to transition back to living in the community. The Transition Specialist will spend their work time in CIDNY’s Queens, NY office and traveling between boroughs.

Veteran Direct Care Independent Living Specialist
The Independent Living Specialist will report to the Director of Employment Vocational Rehabilitation Services and will work collaboratively as well with other programs at CIDNY. The Independent Living Specialist’s key role is to assist the Veterans with disabilities who are home bound and their direct care workers to execute activities of daily living tasks (ADL’s). Travel across the boroughs will be required.
